\n\nRegistered Nurse Qualifications\n

• At least 12 months of recent Home Health RN experience (within the past 3 years)\n
• Proficiency with HCHB (Homecare Homebase) software – required\n
• OASIS documentation experience – required\n
• Active Wisconsin RN License or Compact RN License (must be current)\n
• Current BLS certification\n

\n\nHome Health Nurse Responsibilities\n

• Provide skilled nursing care to patients in their homes across Arbor Vitae, Minocqua, and Lac Du Flambeau\n
• Complete accurate and timely clinical documentation using HCHB and OASIS\n
• Collaborate with interdisciplinary healthcare professionals to optimize patient outcomes\n
• Educate patients and families on disease management, medications, and personalized care plans\n
• Travel between patient homes within the assigned Wisconsin territory\n
