Job Overview: The Senior Case Manager will have the opportunity to join a growing, innovative company focused on creating opportunities and connections for adults with intellectual and/or developmental disabilities (I/DD) to experience rich, meaningful lives and futures.

Responsibilities and Duties:

Case Management Leadership:

  • Oversee and manage a caseload of clients with a focus on high-complexity cases.
  • Partner with onboarding teams, supervisors, families, and clients to build and implement customized support plans.
  • Provide strategic guidance to junior case managers and support staff, ensuring alignment with organizational goals and high-quality client outcomes.

Client and Family Support:

  • Conduct in-depth, hands-on case management to provide direct support to adult clients and their families.
  • Visit clients in-person weekly (at home and in the community) and engage in social and community-based events.
  • Cultivate and strengthen community connections, building lasting relationships with professionals, friends, and natural supports.
  • Identify and apply for relevant employment, internship, and volunteer opportunities.

Advanced Coaching & Advocacy:

  • Provide high-level coaching in executive functioning, social skills, and life skills, including specialized strategies for clients with complex needs.
  • Advocate for clients' medical needs, coordinating appointments and follow-ups with healthcare providers.
  • Provide expert guidance and advocacy in navigating state and federal benefits (OPWDD, Self Direction, Medicaid, SSI, etc.).
  • Support families with housing solutions, roommate matching, and Live-In Caregiver coordination.

Family Coaching & Future Planning:

  • Offer advanced family coaching, conflict resolution, and mediation when needed.
  • Provide strategic partnership and guidance in long-term future planning for clients and families.

Documentation and Reporting:

  • Write detailed, accurate case notes and maintain thorough documentation in compliance with organizational standards.
  • Participate in individual and group supervision, contributing to team development and knowledge-sharing.
  • Attend ongoing professional development training sessions.

Qualifications and Characteristics of the Ideal Candidate:

Education: Master’s Degree in Social Work, Mental Health Counseling, Occupational Therapy, Speech-Language Pathology, or other related fields.

Experience:

  • Minimum 5 years of case management experience , with at least 2 years in a senior or leadership role.
  • Expertise in working with adults with I/DD and their families.
  • In-depth knowledge of state and federal benefits and entitlements (OPWDD, Self Direction, Medicaid, SSI, SSDI).
  • Proven ability to develop and implement person-centered, creative solutions.
  • Experience in supportive and/or affordable housing solutions is highly preferred.
  • Strong leadership skills, including the ability to mentor and guide junior team members.

Job Type : Full-time

Pay : $80,000.00 - $100,000.00 per year
