Codex is backed by Samsara BioCapital. Codex is actively looking for a full time Health Coach to join our talented team. As a Health Coach , you will guide patients through Codex’s cardiometabolic disease management programs. You will be supported by Codex’s provider application and patient app to help track patient progress. Your primary responsibilities will include communicating one-on-one with patients through our platform and collaborating with our clinical care team in order to ensure comprehensive care. What you will do Guide and coach patients through Codex's full suite of Virtual Cardiometabolic Programs—Hypertension, Type 2 Diabetes, and Obesity—ensuring a smooth, engaging, and supportive onboarding experience. Engage with patients through one-on-one video consultations and asynchronously through secure electronic messaging. Utilize motivational interviewing techniques to empower patients to leverage their insights, strengths, and resources to define meaningful goals, create actionable steps, and build a framework for accountability. Motivate and empower patients to achieve beyond what they might accomplish on their own, fostering growth and confidence in their abilities. Provide patients with evidence-based resources and information to support informed decision-making and effective health management. Collaborate as an integral part of a dedicated "teamlet," consisting of a licensed healthcare professional and a health coach, to provide personalized care for an assigned panel of patients. Leverage strong communication skills to effectively collaborate with clinical staff and ensure coordinated patient care. Actively collaborate with the product team to support continued improvements towards scaling care. What we’d like you to bring to the table Bachelor’s degree in a health-related field (e.g., Nutrition, Public Health, Exercise Physiology, Psychology, Health Science, Health Education) is required. A patient-first mindset with an empathetic, judgment-free communication style. Minimum 3 years of experience coaching members one-on-one with health goals in a collaborative healthcare setting. Health coaching certification is preferred but not required. Demonstrated proficiency with digital health applications, web-based software tools, and technology-driven platforms, with experience delivering health coaching through these mediums. Ability to work independently, take initiative, and be resourceful. Familiarity with HIPAA guidelines and a commitment to maintaining patient confidentiality and compliance. Proactive mindset with a "go-getter" attitude, always looking for ways to improve processes and outcomes. Very comfortable with technology and able to troubleshoot basic issues independently.
