What You’ll Do Support individuals with developmental disabilities through person-centered planning. Assist clients with daily living skills, goal-setting, and accessing community resources. Conduct basic assessments to identify strengths, needs, and personal goals. Collaborate with families, service providers, and interdisciplinary teams. Advocate for clients to ensure their voices are heard and their rights are respected. Maintain accurate documentation and case notes. Participate in team meetings and training to strengthen your skills. What We’re Looking For High School Diploma or equivalent (training is provided—no prior case management experience required). Compassion, patience, and cultural sensitivity when working with diverse communities. Strong communication and teamwork skills. Reliable transportation for community visits. Familiarity with local resources is a plus. Experience supporting individuals with developmental disabilities is helpful but not required—we’ll train you!
